Procedure of Taking Oral Temperature
1) To prevent the cross infection wash your hands clean, then collect all clean articles. Take the tray at right side of the patient. So that
a) It will be easy to work.
b) Patient will not get disturbed,
c) It also save the time and energy.
2) Explain the procedure to the patient and relatives to gain their co-operation. Give them an idea that you are taking the temperature by mouth.
3) Pick up the thermometer from the test tube, wipe it with dry swab in a circular movement from bulb to stem, means from sterile area to unsterile area. As portion of the thermometer sinked in lotion is sterile but the remaining portion of the stem is unsterile. The circular movement cleans it from all sides.
4) Ask the patient to open the mouth. Place the bulb of the thermometer under the tongue. Ask him to hold the thermometer firmly by closing the lips. It will avoid the breakage of the thermometer. Observe the  pulse and respiration.
5) Keep three fingers of your right hand on his/her radial artery and count the pulse for one minute then count his/her respiration for one minute.
6) Remove the thermometer from mouth. Do not hold it at bulb side. As mercury may rise up due to your body temperature.
7) Clean the thermometer with dry swab, take it at your eye level by holding horizontal and read the temperature. Do not shake it immediately as there is a possibility of spreading micro-organisms in the air.

Procedure of Taking Axillary Temperature
1) To prevent the cross infection wash your hands clean. Then collect all clean articles. Take the tray at the right side of the patient so that it will be easy to work and it will not disturb the patient and also it will save time and energy,
2) Explain the procedure to the patient and relatives to gain their co-operation. Give them an idea that you are taking temperature by axilla.
3) Pick up the thermometer from the test tube, wipe it with dry swab in circular movement form bulb to stem to maintain the sterilization of thermometer. Because the portion of the thermometer sinked in lotion is sterile but the remaining portion of the stem is unsterile. The circular movement cleans it from all sides;
4) First dry/wipe the axilla with towel as it is moist due to sweat.
5) Keep the bulb of the thermometer in axilla and ask the patient to hold it firmly.
6) Make it sure that the bulb of the thermometer is in the contact with the skin from all sides.
7) Count the pulse for a minute and respiration for one minute.
8) Remove the thermometer from axilla. Do not hold at the bulb side because mercury may rise up due to your body temperature.
9) Take the thermometer at your eye-level. Read the temperature. Do not shake it immediately as mere is a possibility of spreading micro-organisms in the air.

3) Give left lateral position. It makes the nurse to do the procedure easy and the patient also gets comfort.
4) Before you take the temperature of an infant clean the anus with cotton swab if necessary.
5) Take the thermometer from lotion wipe it with dry swab in circular movement from bulb to stem to maintain the sterilization of bulb. The circular movement clean the thermometer from all sides.
6) To avoid the injury due to friction apply vaseline to the bulb of the thermometer as a lubricant.
7) Insert the thermometer quickly but gently 1" to 11/4" inside the anal canal.
8) Count the pulse for one minute and respiration for one minute.
